Ready to set up your IUSD Patient Portal?

 

  • You must have a patient record.

  • We must have an email address on file for you.

  • Use the Register for Access link found below the Login button.

 

 

Important Information

 

         When registering, use your LEGAL name, not preferred or nickname(s).

         Your username will be assigned to you during the registration process.

         The username is case sensitive: Please pay attention to the upper/lower case you enter for your username.

         You will also choose a password (also case sensitive) and some security questions during the registration process.


Q. What do I need to do to have a Teledentistry Appointment?

Sign the eConsent Form

If this is your first log-in to the portal click the iForms menu above. Click the eConsent For Teledentistry form. Read the consent. Type your full name at the bottom of the form and fill out the date. Then click the FINISHED button.

How to Locate Zoom Meeting for Teledentistry

Click the Messages menu and select the message titled Teledentistry Zoom appointment.

Click the link within the message to start Zoom. See example below.

Q. What can I do with IUSD PatientAccess Portal?

·         View your personal information; medical information; insurance policy   information; and a summary of your health information.

·         View a list of your appointments.

·         Complete forms on-line.

 

Q. How can I get access to IUSD PatientAccess Portal?

·         Provide your e-mail address to the registration person at the Dental School when   you set up your patient appointment.

·         A registration will be sent to the email that you provided, giving you instructions   on how to register for access to the IUSD PatientAccess Portal.

·         Click on "Register for Access" in the login box.

·         Enter your birth date, first name, and e-mail address.

 

Q. Can the IUSD PatientAccess Portal be used for minor children?

      Children 17 and younger will not be granted an account to access the information.  Legal guardians can use the IUSD PatientAccess Portal.  Ask the registration person to configure access to each child.

 

Q. How do I change my password?

•  Go into the Settings menu found along the top when you log in; then choose      "Change Password" along the left.

 

Q. I forgot my password. How do I reset?

•  On the My Dental Portal home page there is a “Forgot Password” link under      the Log In tab just below the username and password fields. If you are              unable to use this form to reset your password, please contact us.

 

Q. I need to cancel or reschedule my appointment.  What do I do?

       •     Call our Patient Admitting staff at (317) 274-7433 to cancel or reschedule.

 

Q.  I have radiographs from my private practice office.  How do I get these to you?

       •     Email radiographs to medrecor@iupui.edu

 

Q.  I need my dental records.  How do I get my records?

       •     Please contact the Office of Clinical Affairs at (317) 274-7433.  The staff                  can email you a Release of Information form.

 

Q.  What is your fax number?

       •  317-278-6958

 

Q.    I have a dental emergency.  What do I do?

        •  IUSD Emergency Clinic is open Monday – Friday.  Intake times are 8:00                    a.m. and 12:30 p.m.  For more 

             Information, please call 317-274-7433.

 

Q.    I have a dental emergency after business hours or on a holiday. What do I do?

        •  Call (317) 944-5000 and ask for the dental resident on call.

 

Q.    I am having technical issues with the IUSD Patient Portal?

        •  Emal iusdwp@iu.edu This email is not regularly monitored. 

  •  Please allow 24-48 hours for a response.
